Admissions Procedure

Pre-admission Procedure

Social Workers or Care Managers visit the particular home to view facilities and meet the staff.

Social Workers or Care Managers provide us with the latest detailed clinical report of the individual.

We visit potential clients at their current placement.

If Social Workers or Care Managers consider that Truecare are able to offer a suitable placement, potential clients visit the chosen location for themselves.

When the potential client confirms that they are happy to proceed an admissions meeting is arranged to plan the individual arrangements.
One aspect covered by this meeting is to allocate a key worker who will be that client's personal mentor.

Post-admission Procedure

Individuals are admitted on a six month trial period, during which time further assessment of their needs is carried out by qualified staff and ongoing placement suitability is determined.
A formal care plan is developed which forms the basis of all subsequent reviews.

If at any time during the trial period the service user persistently wishes to leave or is considered unsuitable, he or she should be able to return to their previous accommodation or suitable alternative.

Financial Arrangements

All service users are charged the appropriate and agreed fee. Individuals are entitled to relevant personal allowances with personal monies being paid into their own individual accounts.

Keyworkers assist service users in making appropriate use of their own funds.
